Second redo surgery after two consecutive failures of a colorectal or coloanal anastomosis: is it reasonable?

چکیده
Colorectal redo surgery is well known to be a difficult procedure, associated with high risk of failure. The aim this study was look into patients presenting two consecutive failed colorectal (CRA) or coloanal (CAA) anastomosis who underwent second (i.e., third anastomosis). A retrospective based on prospective database surgeries CRA CAA, in an expert center. Sixteen between 2005 and 2020 were analyzed. After mean follow-up 28 ± 26 months, success (defined as no stoma at the end follow-up) reported 10/16 (63%). One patient chronic anastomotic leakage another early colonic ischemia had defunctioning reversal. In remaining four surgery, definitive ultimately created for fistula recurrence (n = 1), poor functional results 2), local cancer 1). Two factors failure significantly found univariate analysis: (1) nature primary anastomosis: 3/13 s (23%) if first made 3/3 (100%) it CAA (p 0.036); (2) age: older 0.04). 63% rate observed after CAA. Although demanding can proposed carefully selected motivated patients.

the bone is “fractured” or “broken”. While orthopaedic surgeons consider these terms synonymous, patients appear tocomprehend the terms as having different meanings. Given the commonality of this frequently posed question, it may beimportant for providers to assess patients’ level of understanding in order to provide optimal care. The purpose of thisstudy is to evaluate patients’ comprehension ...
متن کاملLong-term functional results after straight or colonic J-pouch coloanal anastomosis.
Proctectomy followed by straight coloanal anastomosis (CAA) often results in poor functional outcome known as the anterior resection syndrome. It is now based on evidence that a colonic J-pouch CAA improves outcome in the first 2 years. We assessed the very late functional outcome of CAA patients with or without a pouch. These results show that the functional benefit of the J-pouch anastomoses ...
متن کاملLaparoscopic Coloanal Anastomosis for Low Rectal Cancer
OBJECTIVES Low anterior resection with hand-sutured coloanal anastomosis for low rectal cancer is technically feasible, and it does not compromise oncologic results. We describe herein the effectiveness of the operation in treating low rectal cancer by a laparoscopic approach followed by intraanal canal dissection. METHODS From February 1999 to October 1999, we used a laparoscopic procedure t...
متن کاملRecurrent pilonidal disease surgery: Is it second primary or reoperative surgery?
OBJECTIVE Pilonidal sinus disease (PSD) effects mainly young men's social and work life with frequent recurrence rate. Reoperation for unimproved or recurrent disease is somehow troublesome. Surgeons may think that changing treatment strategy after recurrence may prevent further relapses of PSD. We analyzed patients with recurrent pilonidal sinus to determine their predisposing features for rec...
متن کاملTransvaginal coloanal anastomosis after rectal resection for the treatment of a rectovaginal fistula induced by radiation.
Although decreasing in number, radiation induced rectovaginal fistulas are caused by some radiation injuries and chronic ischemic lesions. Most of the experienced authors recommend anterior rectal resection with coloanal anastomosis accessed through the abdominal-perineum area for high fistula. We present a patient with a fistula that developed 23 years after hysterectomy and radiotherapy. In t...
